Please copy and paste the questions below into the body of your e-mail and answer each one.

NOTE:  All information you provide will be kept confidential and NOT given to spammers or
anyone else.

1.  Your name.  For weddings please provide bride & groom name, if other than yourself.  

2.  Your phone number and / or e-mail address (so we can get back to you with a quote).

3.  Your address (in case you decide to book us).

4.  How did you hear about us?  (Google search, WeddingWire.com, AtlantaBridal.com, referral, etc.)

5.  What type of event?  If a wedding, please include all parts you're interested in having us perform (ceremony only, ceremony plus cocktail hour, etc.)

6.  Date of the event.

7.  Time of the event.  If a wedding, please provide the CEREMONY time (same as on your invitation).

8.  Venue / location.   Please include as much of the address as you know.

9.  Is your event indoors or outdoors?

10.  What ensembles would you like quotes for?  Type a capital X next to each one you're interested in:
           * String quartet  
           * String trio     
           * Violin-cello duo  
           * Violin-piano duo 
           * Add trumpet to wedding ceremony  

11.  Are you interested in adding a vocalist to the performance?  If yes, please let us know if it's a friend of yours or if you'd like us to hire somebody for you.  Also let us know what song you'd like to have him/her sing (if known).

12.  Do you have any special music requests (such as a pop tune we may not have in our library)?
